This is a flawed comparison. The fact is that smokers, excessive drinkers and the obese will run into health problems but they also die much younger than others and so avoid the long period when older adults have health problems which costs much more than the obese, alcoholics and smokers.

Studies in, I think it was Australia, found that the early death more than compensated for the extra burden on the healthcare services during their life.

Obesity, alcoholism and smoking are net cost savers due to the much shorter lives of these individuals.
